Welcome

· The best way to complete this Assessment Tool is with a team that includes the Executive Director or another senior manager, a frontline worker, and one or more other people from different parts of your organization. The more diverse the representation, the more accurate your responses will be, and thus the more effective the Tool. For the sake of consistency, each question should be completed by the same people.

· You will need a copy of your agency’s policies and procedures to refer to.

·This Tool is designed to be an anonymous self-assessment.

· Create a user login using the Create New Account button on the right-hand side of the page.

· You will use the same username and password to log in each time you visit the Tool. This way, you can save your work, continue filling it out at a later date, and view past results for comparison.

· This Tool uses a multiple-choice format. The response options are examples of scenarios. For each question, please choose the response that is the closest to describing your agency's present situation.

· The Tool is divided into six sections with varying numbers of questions.

· To navigate between sections use the right-hand menu bar.

· Click Save Draft at the end of each page, BEFORE moving on to the next section.

· You can revisit the section and change your answers later.

· You may click View Results for a completed section in the right-hand menu bar at any time.

· When you are finished, click Submit All for Final Report at the bottom of the right-hand menu.

· You may view these results again at any point by clicking View Results.

· To start a new assessment, simply click Start New Draft on any section in the right-hand menu bar (this will not affect previous Results pages).

· Click View Results in the right-hand menu bar to view all of your completed assessments.

· Many of the questions are accompanied by Tips. These Tips appear in your final results as well, and connect you to resources that can help you develop and strengthen your Positive Space.

· You can refer to the Glossary to learn how we define the terms used in this Tool.
